About the Role
This is a dynamic outreach and intake position working directly with Aboriginal young women across Melbourne. The role combines proactive community engagement, crisis response, intake assessment, advocacy and systems navigation. the role reports to the Team Leader and works closely with the Case Manager and support team.
You will:
Conduct outreach at youth hubs, access points, schools and community locations
Build trust using culturally safe, youth-centred and trauma-informed approaches
Complete intake assessments and confirm eligibility for hostel placement
Document risks and protective factors and contribute to safety planning
Facilitate crisis accommodation referrals where hostel placement is not suitable or unavailable
Coordinate warm referrals and practical supports such as transport and appointment attendance
Maintain accurate and confidential records
Participate in supervision, reflective practice and continuous improvement
About You
You are a confident outreach practitioner who understands the complexities of youth homelessness and the importance of culturally safe engagement.
You will bring:
Demonstrated understanding of Aboriginal and/or Torres Strait Islander culture
Experience supporting Aboriginal young people using trauma-informed practice
Knowledge of homelessness systems, crisis accommodation and youth AOD/mental health pathways
Strong skills in outreach engagement, assessment, advocacy and safety planning
Ability to collaborate with ACCOs and mainstream services
Excellent communication and documentation skills
A current Driver’s Licence and willingness to travel across metro Melbourne
Qualifications in Youth Work, Social Work or Community Services are desirable.
